Significant Changes:

• Breastfeeding problems — reviewed. A literature search was conducted in May 2026 to identify evidence-based guidelines, UK policy, systematic reviews, and key randomized controlled trials published since the last revision of the topic. No major changes to recommendations have been made.
• Constipation — reviewed. A literature search was conducted in June 2026 to identify evidence-based guidelines, UK policy, systematic reviews, and key randomized controlled trials published since the last revision of the topic. Information on prescribing in pregnancy and breastfeeding has been updated and incorporated into the prescribing information section, the section on Factors affecting choice of laxatives has been incorporated into the Choice of laxatives section, and details on dietary recommendations have been updated and aligned with the British Dietetic Association Guidelines for the dietary management of chronic constipation in adults.
• Contraception – assessment — reviewed. A literature search was conducted in June 2026 to identify evidence-based guidelines, UK policy, systematic reviews, and key randomized controlled trials published since the last revision of the topic. The topic has been aligned with the 2025 College of Sexual and Reproductive Health (CoSRH) UK Medical Eligibility Criteria for Contraceptive Use (UKMEC).
• Head injury — reviewed. A literature search was conducted in June 2026 to identify evidence-based guidelines, UK policy, systematic reviews, and key randomized controlled trials published since the last revision of the topic. There have been minor structural changes to the topic, including the addition of new background information sections summarizing prevalent causes and risk factors related to head injury. Guidance about considering further investigations or referral for people with persistent symptoms consistent with hypopituitarism have been added to the topic, to align with updated NICE guidance. Guidance about returning to sport, work and school post-injury has been updated to align with the 2022 International Consensus Statement on Concussion in Sports and UK Government guidance.
• Palliative care – General issue — reviewed. A literature search was conducted in June 2026 to identify evidence-based guidelines, UK policy, systematic reviews, and key randomized controlled trials published since the last revision of this topic. The Scenario on Communication has been deleted and the content incorporated into the Scenarios on ‘Assessment and management approach’ and ‘The terminal phase’. Various web links to patient and carer information have been added to the topic.
• Stroke and TIA — reviewed. A literature search was conducted in May 2026 to identify evidence-based guidelines, UK policy, systematic reviews, and key randomized controlled trials published since the last revision of this topic. The topic has been updated in line with the updated National Institute for Health and Care Excellence (NICE) guideline Stroke and transient ischaemic attack in over 16s: diagnosis and initial management [NICE, 2025] and the Intercollegiate Stroke Working Party (ICSWP) publication National clinical guideline for stroke for the United Kingdom and Ireland. Modified-release dipyridamole is no longer recommended as a drug option for antiplatelet therapy. The recommended indications, doses, and duration of use for dual antiplatelet therapy (DAPT) have been amended in line with current evidence in the literature.

Minor Changes:

• Diabetes type 1 — minor update. QOF indicator table updated for 2026/7.
• Diabetes type 2 — minor update. Information in the prescribing section regarding empagliflozin and dapagliflozin has been updated to be in line with NICE guidelines.
• Erectile dysfunction — minor update. Amended to show generic prescribing of PDE-5 inhibitors is without restriction in the NHS.
• Hypertension — minor update. QOF indicator table updated for 2026/27.
• Menorrhagia — minor update. The recommendation advising against routine serum ferritin testing has been removed in line with NICE guideline NG88.
• NAFLD — minor update. Names changed to metabolic dysfunction-associated steatotic liver disease (MASLD) and metabolic dysfunction-associated steatohepatitis (MASH).
• Neuropathic pain — minor update. The availability of capsaicin cream was amended.
• Obesity— minor update. New QOF indicators for Obesity for 2026/27 have been added.
• PAD — minor update. Removed table of ABPI thresholds and added PAD threshold for PAD diagnosis.
