A well recognised, yet notoriously difficult to prevent, subset of these incidents are Look-Alike, Sound-Alike or LASA-errors.

  • Recognise some of the causes of Look-Alike, Sound-Alike medication errors.
  • Demonstrate how to use Agilio to establish a baseline treatment pathway.
  • Identify potential drug-induced causes of medical conditions using Martindale.
  • Utilise the BNF to perform an ‘indication check’ if there is a suspicion of LASA errors.
  • Evaluate drug-drug interactions between acute treatments and long-term medications using Stockley’s Interactions Checker.
