New dosing regimen for fenofibrate in adults with diabetic retinopathy.
The fenofibrate monograph in BNF now includes a new indication and dosing for non-proliferative diabetic retinopathy in type 2 diabetes. This supports a recommendation in NICE guideline NG242 ‘Diabetic retinopathy: management and monitoring’, which advises that fenofibrate should be considered in these patients to reduce progression of the condition.
Our editorial team appraised primary literature to find relevant doses and consulted with experts from The Royal College of Ophthalmologists and the Association of British Clinical Diabetologists. This guided the dosing and appropriate level of supervision required for the initiation of this unlicensed use of fenofibrate.
The new fenofibrate dosing regimen is unique to the BNF and has been ratified by the Joint Formulary Committee. Fenofibrate has also been noted as a treatment option for diabetic retinopathy in the treatment summary for Diabetic complications. Updates to BNF will be published for the July 2026 update for online and app versions.
